The Affordable Care Act (ACA) has many health care institutions putting care transition teams in place and working with community providers.  But not all community agencies understand the overall picture and how ACA may affect them… more importantly how can and should community agencies be part of the planning and delivery.

The goal of this conference is to educate service providers serving diverse elders on the Affordable Care Act and its potential impact on service delivery.
