I Am A Survivor, Now What?:
Staying Healthy, Happy, and Productive after Cancer

The Nemours Children’s Clinic, Jacksonville Oncology department has received a grant from the American Cancer Society, Inc., Florida Division to hold a conference for young adult cancer survivors. Childhood cancer survivors from the ages of 16 to 29, along with their spouse or significant other are invited to attend. This conference will provide participants with knowledge about late effects seen in childhood cancer survivors. There will be 6 speakers covering various topics including an overview of late effects of cancer treatment, higher education and learning issues experienced by cancer survivors, vocational opportunities, psychosocial aspects of survivorship, healthy living, navigating the healthcare system, and financial literacy. Small group support and networking will also be provided.

Sean Swarner

2X Cancer Survivor, Mountain Climber, Author

 Sean, a two-time pediatric cancer survivor, will share his amazing story of triumph over cancer. He is the first cancer survivor to climb Mount Everest with only partial use of his lungs. He continued to climb and has completed the ‘7 summits’.  He is also an author and motivational speaker sharing his message of healing, hope and triumph with cancer patients worldwide. ***Participants living greater than 50 miles from the venue will be offered reimbursement for a 1 night stay at the Omni Hotel, 245 Water St, Jacksonville, FL for the evening of Friday, 11/15/2013 prior to the conference.  This special rate is for a limited block of rooms only. First come, first served until the block is filled.   Rooms booked beyond the block will not be reimbursed so book your hotel early.


 **IMPORTANT: Instructions for reimbursement below**


  1. Bring a copy of your receipt received at check-out to the conference showing room fee and taxes for 1 nights stay 11/16/2013. (sorry no other charges or additional nights will be reimbursed).
  2. Checks to only be mailed to address listed during registration within 30 days.
  3. Rooms booked outside the Omni or after our block of reserved rooms is filled will not be reimbursed.
