American Home Health and Hospice Inc.
Al Bracken is currently the Vice President of Business Development for American Home Health and the owner of Integrated Support Services Inc. (ISS) of Indianapolis. ISS is a new venture that specializes in providing housing solutions with comprehensive medical services for seniors and homeless veterans. The ISS model is comprehensive in nature based on providing services across the continuum, including pharmacy, Home Health care, and medical services. Al is also a 29 year veteran of the Air Force/Indiana Air National Guard where he is currently the Inspector General at the 122 Fighter Wing in Fort Wayne, Indiana. Al has over 26 years’ experience in the pharmaceutical industry with 15 of those years dedicated to providing medical nutritionals and pharmaceutical services for the unique needs of our geriatric population. He has also served on various tasks forces and committees through-out the years with programs such as the Retired Senior Volunteer Program (RSVP), Meals on Wheels, the Senior Companion program, AARP, various Community Development Corporations (CDCs) congregate meal sites, the Gary Community Health Foundation, University of Indianapolis Center for Aging and Community, The Damien Center, and various church organizations through-out Indiana and the mid-west. He is currently a Board member of the Indiana Minority Health Coalition, the Tuskegee Airman Inc. and a member of the American Legion. He also serves on the Indiana Adult Guardianship taskforce and The Indiana Home Health taskforce.
Annette Seabrook, MPT, MHA, FACHE

Annette Seabrook is Program Director for the Inpatient Rehabilitation Center at Franciscan St. Francis Health. She began her career at Schwab Rehabilitation Hospital in Chicago, followed by 15 years at Rehabilitation Hospital of Indiana (RHI) in Indianapolis before transitioning to Franciscan St. Francis Health in 2010. Originally from central Illinois, she received her B.A. from Augustana College, where she was also a two-sport athlete. She went on to obtain her Master of Physical Therapy from Hahnemann University in Philadelphia and Master of Healthcare Administration from IUPUI. She is a Fellow of the American College of Healthcare Executives and founder of the Indiana Spinal Cord and Brain Injury Research Fund. Currently, she serves on the Advisory Board for Indiana Healthcare Executive Network (IHEN); Regent Advisory Council (ACHE); IU Richard Fairbanks School of Public Health Alumni Association Board; and Chair, Indiana Hospital Association Rehab Task Force. An avid Colts and Pacers fan, she lives on the northeast side of Indianapolis with her husband and border-collie mix, Halle.

Brian Holmes has over 10 years of experience working in the pharmaceutical industry in sales. He has been involved with the promotion of pharmaceuticals to hospitals, physicians, residents and pharmacies. He has trained other sales representatives on new sales initiatives and new pharmaceutical applications. Brain has been responsible for establishing relationships with key decision makers for the promotion of promoted products. Brian has also been a faculty member with the University of Phoenix since 2009, and he has been the Lead Faculty Area Chair for the College of Health Science and Nursing for over two years. Brian has made a positive impact on our students through teaching and coaching other faculty. He is currently working on his PhD in Public Health.

Darnae’ Scales has over 20 years of diverse social services experience. His experience includes mental health therapy, hospital social work, delinquency prevention, faith based social services, grant writing, and community organizing. He is a Licensed Clinical Social Worker in private practice at Lives Transforming Counseling in Fishers, Indiana and for the past 7 years has served as the Director of JEWEL Human Services, Inc. Mr. Scales earned a Bachelor of Arts degree in Sociology at Indiana University; a Master of Arts degree in Social Service Administration at The University of Chicago and a Graduate Certificate in Health Administration & Policy—The Harris School of Public Policy (UChicago). He is also a Registered Domestic Relations Mediator in the State of Indiana and a member of the National Association of Social Workers (NASW-Indiana Chapter).
Jim Leich, MPAff

Jim Leich has been the President and CEO of the LeadingAge Indiana (formerly Indiana Association of Homes & Services for the Aging) for the last 18 years. LeadingAge Indiana represents 180 nursing homes, retirement communities, hospital-based skilled units, assisted living facilities, and senior housing projects. LeadingAge Indiana provides a variety of educational, informational, governmental relations, and lobbying services for its nonprofit members.
During his time at LeadingAge Indiana, Jim has served on numerous state advisory committees and task forces, including the Governor’s Commission on Home and Community Based Services and the Indiana Adult Guardianship State Task Force which he currently chairs. He is past chair of the LeadingAge-State Affiliate Executive Forum (national organization) and was a member of the LeadingAge Long Term Care Financing Cabinet. He has also served as the President of the Indiana University School of Public and Environmental Affairs Alumni Board and as a member of the Indiana University Alumni Association Executive Council.
Prior to joining LeadingAge Indiana, Jim was the Director of the Indiana Long Term Care Program, an innovative partnership between the Indiana Medicaid Program and private long-term care insurance. In addition, Jim has served as a senior policy analyst for the Indiana Legislature and as Vice-President of Dixon and Associates, Ltd., a consulting firm serving the long-term care and senior living industry.
Patrick Broccolo, CNA

Patrick Broccolo, CNA is co-owner of Senior1Care. Senior1Care is a homecare agency specializing in the management of chronic diseases, in particular Alzheimer’s disease, and the coordination with healthcare providers to assure quality outcomes by preventing unnecessary hospital visits. Patrick graduated from Butler University in Marketing and has lived in the greater Indianapolis area for many years. After several years working in the marketing field, Patrick joined with his family’s business after assisting his mother-in-law through a battle with cancer. He became a licensed Certified Nurse Aide and started the Carmel office in 2008. Throughout his career, he has learned the value of professionalism and quality service which are important values in working with families and the healthcare system. Patrick is a board member of the Central Indiana Case Management Society of America chapter, and he is a member of the Community Service committee at the Rotary Club of Indianapolis.
