Description

Patient transmission of care creates an opportunity for miscommunication. The SBAR transition of care tool provides a standard method of communicating to decrease errors or loss of information during this crucial transition.

Intended Audience includes: Any medical caregiver, including residents and students, who are responsible for the transfer of patient care within any specialty.

The program objectives include:

  • Review the SBAR transition of care mechanism for patient communication;
  • Recognize the high level of importance of accurate patient care sign-offs;
  • Reveiw the ACGME/CLER/Joint Commission level of concern for sign-offs; and
  • Review the SBAR sign-off form/perform mock sign-out for learning/critique.
