Case Manager in Home, Nurse in Pocket

Care managers in the home using mobile technology called Care at Hand have reduced hospital readmissions among at-risk patients, improved care and

demonstrated savings.

Presenters: Andrey Ostrovsky, MD, CEO Co-Founder Care at Hand Boston University School of Medicine.  Lori O’Connor, RN, Director of Nursing at Elder Services of Merrimack Valley.

Reuse of expensive assistive devices and durable medical equipment just makes sense.  Learn free and low cost ways to find or donate unused equipment.

Presenter: Randi Sargent, MBA, BS, REquipment Program Manager, The Boston Home, the MRC Assistive Technology Program (MassMATCH).

 

 

Hoarding Disorder: Clearing Paths to Collaborative Resolutions

Understanding Hoarding Disorder through experiential exercises and discussion.  Learn how to use empathy, empowerment, and engagement to collaboratively address and work to resolving hoarding happenings.

Presenter: Marnie McDonald, MSW, LCSW, is the Program Director and Clinician for North Shore Hoarding Resource Center at North Shore Elder Services.

 

 

Mental Health Services for Older Adults Confined to Home

There is increasing demand for mental health services for older adults confined to home.  This presentation will review trends in mental health services and how to meet the need through increasing capacity in evidence-based programs.

Money Management and Financial Literacy

Learn the benefits of financial education for older adults.  Included will be a review of “Finances 50+”, a program designed specifically for older adults and information on how to avoid scams and financial exploitation.

Presenter: Cheryl Cannon, Statewide Coordinator, Massachusetts Money Management Program.
