This activity has been designed to meet the educational needs of Registered Nurses, Social Workers and other Healthcare Professionals involved in the care of patients in long-term care facilities.
The healthcare professional must be cognizant of their respective roles within the multidisciplinary team to ensure safety and the provision of optimal care to geriatric and developmentally disabled individuals. The nurse, physician, pharmacist or other medical professional must ensure that they are utilizing best practices in documentation, assessment and treatment of individuals. They are to receive specific training and education regarding medical/mental health conditions and risk indicators of abuse and neglect that are common among the senior and special needs populations. They are to have sound knowledge of medications and potential side effects that are compounded by age or a dual-diagnosis. The healthcare professional, subsequent to participation in this activity, will be able to apply acquired knowledge to their current practice to improve performance, develop more effective communication techniques, ensure proper documentation, and strengthen their function as a member of the multi-disciplinary team. This will ensure effective treatment of individuals and allow for better patient outcomes.
After completing this activity, the participant should be better able to:
- Develop appropriate strategies and protocols to assess for risk of abuse and neglect in the senior and developmentally disabled individual
- Employ an interdisciplinary model in the provision of care to geriatric and dually diagnosed individuals
- Apply techniques to enhance the quality of documentation and communication amongst members of the multi-disciplinary team
- Perform routine screenings of all individuals at high risk for abuse and neglect
- Recognize a dual diagnosis and incorporate techniques into current practice to provide treatment methods that are appropriate for the individual with co-occurring developmental and mental health conditions
- Manage and monitor administration of medication and recognize potential side effects common amongst the dually diagnosed population
- Develop and implement a variety of strategies—behavior and environmental modifications, counseling, psychopharmacology— to improve outcomes of dually diagnosed individuals
- Provide appropriate care and counsel for patients and their families

June Crawford, RN, BSOM, CHC, Principal, The Bonadio Group
June is a Principal in the Compliance Solutions Division at The Bonadio Group. She is a Registered Professional Nurse with more than 25 years of supervisory experience in acute and long-term care settings, and home health care. For fifteen years, June held the positions of Director of Nursing Services, Director of Residential Services, and Director of Continuous Improvement with a large not-for-profit agency in the field of Developmental Disabilities. She has also served as Corporate Compliance Officer for two health and human service agencies.
In addition to her clinical background, June holds a Bachelor of Science Degree in Organizational Management. She has extensive experience in organizational management including strategic planning, regulatory compliance, Medicaid and Medicare compliance, administrative investigations, performance management, team building, continuous quality improvement, corporate compliance, HIPAA implementation, and policy development. June’s expertise in the health and human service sectors has provided her with the ability to transfer that knowledge and experience to assist a broad array of provider types including hospitals, nursing homes, human service agencies, county governments, physician practices, clinics, home health care, and school districts. She is a frequent presenter at state and national conferences and provides training on such topics as Corporate Compliance, HIPAA Privacy, and Documentation Requirements.
June has received certification in Healthcare Compliance, MDS and OPWDD Abuse, Death and Administrative Investigations. She serves as a member of the NYS Surrogate Decision Making Committee and worked with NY Office of Medicaid Inspector General as an Advisory Committee Member in 2010-2011. She is a member of NYS Association of Community and Residential Administrators (NYSACRA), New York State Rehabilitation Association (NYSRA), Health Care Compliance Association (HCCA), and Healthcare Financial Management Association (HFMA), is active in hospice, and serves as a Board Member of several non-profit organizations.

A. Long-term care: The Role of the Consulting Pharmacist
Presented by Arthur Naliboff, RPH, Director of Pharmacy and Compliance, Emblem Health
Arthur Naliboff has been with Emblem Health since 1992 and since 2006 has been the Director of Pharmacy Compliance and Audit. Mr. Naliboff has an M.S. in Hospital Pharmacy Administration from A&M Schwartz College of Pharmacy, as well as a B.S. in Chemistry from Long Island University and a B.S. in Pharmacy from A&M Schwartz College of Pharmacy at Long Island University.
This presentation will analyze the role of the consulting pharmacist as a member of a comprehensive interdisciplinary team caring for patients in long-term care facilities. It will discuss the demographic changes that have lead to the increased demand for pharmacists who work with long-term care facilities. It will also discuss the goals the consulting pharmacist seeks to achieve when providing care and the issues they must take into account when treating the long-term care population. The presentation will review requisite knowledge and skills the consulting pharmacist must master in order to successfully work in this interdisciplinary team environment.
B. Chronic Disease Management in the Elderly
Presented by Mary Ann Rosa, NP, Assistant Professor of Nursing, Queensborough Community College
An RN since 1982 with experience in medical – surgical nursing, telemetry, emergency room, and community nursing . Roles in community nursing include visiting nurse, manager, educator and program developer. A Gerontological Nurse Practitioner since 1996 with experience in an angioplasty program, cardiology private practice, extended care, and program development. In 1999, developed the North Shore LIJ Heart Failure Home Care Disease Management Program. In 2000, set up the Telehealth Program at NSLIJ. Worked full time with these programs until 2006. 2006 took a full time position teaching at Queensborough Community College teaching Community Health and Advanced Medical Surgical Nursing. In 2010, became the Coordinator for NU 202 and Community Nursing at QCC. Since 2006, works on a part time / per diem basis providing education/ consultation for the NSLIJ Heart Failure Home Care Program and assisting in the development of a Diabetes Disease Management Home Care Program in 2012. Is currently working toward becoming a Certified Diabetes Educator. Presented nationally at the National Association of Home Care Annual Conference held in Albany in 2000, and LA in 2009 on Heart Failure Disease Management and Telehealth. Presented a national webinar on Heart Failure Disease Management for Pesi in February 2012 and will be doing a repeat webinar in 2013. Has presented at numerous locations locally on Disease Management. Had an article published by Home Healthcare Nurse in 2008 on How a Heart Failure Home Care Disease Management Program Makes a Difference. Currently working on another article for publication on Disease Management.
Approximately 20-25% of patients 65 years and older, discharged from the hospital, are rehospitalized in 30 days. (N. Engl J Med, 2009; 360 (14);1418) A substantial number of these hospitalizations are avoidable. Hospitals are not reimbursed when a patient returns within 30 days. Circulatory diseases, particularly heart failure, and diabetes account for the majority of these hospitalizations (CDC). Evidence tells us that intensive disease management interventions can prevent these hospitalizations (CMS). There is a national initiative to decrease avoidable hospitalizations, and therefore, decrease costs and increase quality of care and life in our communities. Disease Management is a collaborative, interdisciplinary approach which promotes client centered care and client and caregiver self-empowerment. It also helps to eliminate health disparities and increase access to care. Health providers need to be knowledgeable about how to implement disease management and work toward the overall goal of improving the health of our communities. Health care providers need to know the most common chronic illnesses, the standards of care, and why patients get rehospitalized. Older adults and patients with cognitive impairment require specialized care. Thorough assessments, communication, documentation, and collaboration need to be performed. This increases continuity of care across the continuum of care. Health care providers need to understand the psychosocial component of disease management and understand how to employ behavioral strategies. Efficient client education, based on cultural beliefs, will facilitate client and caregiver self-empowerment. Through Proper Disease Management, avoidable hospitalizations and health care costs will decrease. In addition, patient and health care provider satisfaction, and quality of life will increase.
C. Survey Preparation for Nurses and other Clinical Staff
Presented by: Frederick Wetzel, Ph.D., Independent Consultant
Frederick Wetzel, Ph.D.., is the former Downstate Director of the New York State OPWDD, Division of Quality Assurance/Bureau of Program Certification. His career with OPWDD spanned 31 years, during which he held positions as a Case Manager, Technical Assistant, Surveyor, Regional Director, and Director of Clinical Review. Dr. Wetzel was a member of the OPWDD Nursing Management Team. He chaired the DQM Health Care Committee and the Statewide Committee on Incidents.
Dr Wetzel holds certification as a Surveyor from CMS and is certified as a Legal Nurse Consultant. He is a Senior Member of the American Society for Quality (ASQ) and a Member of the Health Care Compliance Association (HCCA). He is a member of the NYS ID/DD Nurses Association and serves on the committee for the annual nursing conference. He is a frequent presenter at professional conferences.
This workshop will describe methodologies that allow clinical staff to maintain a continuous program of documentation and service provision which will provide evidence of the provision of quality services to the individuals served and supported by their agency, while remaining in compliance with all applicable laws and regulations.
Requirements for Day and Residential services and applicable standards of quality service provision will be described and discussed. Methods for interacting with Surveyors and managing conflict during the survey process will also be discussed.

Survey Protocols and Standards;
Interacting and answering Surveyors questions.
D. Addressing the behavioral and environmental variables that play a significant role in the lives of the developmentally disabled and geriatric individuals
Presented by: Zaur J. Isaakov, Ph.D, BCBA-D, Independent Consultant
Dr. Isaakov received his doctorate in the Psychology of Learning Processes and Applied Behavior Analysis from the Graduate Center of the City University of New York and MA from Queens College. He has been a practitioner of applied behavior analysis (ABA) for the past 11 years in both the home and school settings and has taught courses on the psychology of learning and Autism at CUNY. Dr. Isaakov is a Board Certified Behavior Analyst (BCBA-D).
The presentation will center on research-driven strategies designed to address the behavioral and environmental variables that play a significant role in the lives of the developmentally disabled and geriatric individuals. The participants will be aided in learning to distinguish between behavioral, medical, and psychiatric difficulties an individual may present with; how each of these components, or a combination thereof, may affect their lives. The participants will be introduced to various behavioral strategies that can be effectively utilized to address the behavioral (learned) components of an individual’s life, thus not only improving their overall wellbeing but also allowing medical practitioners to better address the underlying physiological problems.
The participants will learn the ways in which Applied Behavior Analysis strategies may be used to help individuals with developmental disabilities overcome many of the learned behavioral problems and improve their overall quality of life. Those attending the presentation will learn to recognize and assess behavioral patterns associated with neglect or abuse. From the behavioral perspective, in addressing the difficulties faced by those individuals who are dually diagnosed, we will focus on the importance of the function that the illicit substance or alcohol serve to a dually diagnosed individual and explore ways in which safe and appropriate alternatives may be identified.
Throughout the presentation, the findings of both the latest and the now-classic research on the subject will be reviewed along with practical application of these findings to real-life situations.
