Please bring the following items to your appointment:

• Photo ID ( For Taxpayer and Spouse)
• Original (No photo copies) Social Security Cards or ITINs for self, spouse and all dependents (REQUIRED)
• Valid Email Address
• Birth Dates for dependents
• Confirm if self, spouse and dependents are covered by health insurance during 2014
• If insurance was purchased through the marketplace (www.healthcare.gov), bring Form 1095A
• Wage and earning statements (W-2, 1099R, Social Security, etc.)
• Education tuition (Form 1098T)
• Bank check for Direct Deposit information
• Last year’s tax returns, if available
• All other tax documents
