The Region II Centers for Medicare and Medicaid Services (CMS) and ShapingNJ, the Office of Nutrition & Fitness, New Jersey Department of Health cordially invite you to join us for a free one day meeting entitled "Bringing the Affordable Care Act Home: Linking Communities with Prevention and Chronic Disease Management."
This meeting will inform and engage community-based organizations, medical organizations, and other key groups on the impact the Affordable Care Act and HITECH legislation have on prevention, treatment, chronic disease management, coordination of care, and payment reform in New Jersey.
Please join us on Wednesday, May 8, 2013 from 8:00 AM to 5:00 PM, at the Rutgers University Busch Campus Center. A healthy lunch will be provided courtesy of Shaping NJ. Free parking will be available at designated lots with additional information on parking to be available close to the date of the event for those who register in advance.

- Collaborate with community partners from across the State to improve health through technology, enhanced care coordination, and prevention
- Share resources and form partnerships with HRSA, NJ-HITEC, the New Jersey Department of Health, ONC, State and Federal grantees, community organizations, and both the CMS Regional Office and the CMS Innovation Center
- Learn from a panel of experts how electronic health records and delivery system reforms are already working in New Jersey, and share your challenges and knowledge with colleagues and policy leaders in regional breakout sessions
