The money raised will benefit an educational trust fund established for Andrew's 2 young children and the Wolfson’s Pediatric Heart Center in Jacksonville, as well as, other causes Ago Vita identifies.
The cost of the event is $150 per golfer. The cost includes: a box lunch, 2 drink tickets, giveaways and a buffet dinner.
If you would like to attend dinner and not play golf, the cost for dinner is $40 per person.
If you cannot attend, but would like to make a donation, that would be greatly appreciated.
If you are a business owner, or would like to sponsor a golf hole, we have established two levels of sponsorship:
Platinum Sponsor: $300, price includes very prominent signage.
Gold Sponsor: $150, price includes noticeable, but less prominent signage.
Keep in mind, all contributions and sponsorships are fully tax deductible, as Ago Vita is considered a Charitable Organization.

When & Where
Why we exist:
First, we exist to honor a friend and loved one.
Second, Ago Vita, Inc’s primary activity is to further awareness and education of common health issues, such as heart attacks and stroke. Ago Vita will partner with medical professionals and health facilities to educate people of the risks of heart attacks and strokes, as well as encourage proper diagnostic testing to individuals in the purpose of preventing heart attacks and strokes. Ago Vita, Inc., will be on the front end of awareness, by promoting healthy life choices, and health screenings. This will include risk factor evaluation and education on risk factors.
The activity will be conducted by local health organizations, sponsored by Ago Vita, Inc. Initially the organization will be functioning in the Jacksonville, Florida area, but may expand awareness by having an online presence.
Health screenings, and risk factor assessment, will hopefully assist in early detection and life style changes which have been shown to strongly diminish the likelihood of a sudden health crisis in an otherwise healthly individuals.
Initially, 100% of the organizations time will be spent on the awareness and risk factor assessment program.
Funding will come by means of two primary sources. First, online donations to the charity to further the cause of prevention. Second, an annual golf tournament and dinner, that will raise funds for the prevention and screening program.
