Dinner Date: Sunday, September 11th, 2016

Doors open at 6pm Dinner served at 6:30pm

Dinner Event Ticket: $50 per person – open to all individuals

Deadline to purchase dinner tickets: September 2nd, 2016

Registered golfers dinner ticket included in Entry fee for Sept. 11th outing

All golfers are still required to RSVP for dinner even though dinner ticket is included in golf fees.

Golf Outing Date: Monday, September 12, 2016

Location: Sycamore Hills Golf Club, 11836 Covington Road, Fort Wayne, IN 46814

Schedule of Events:

Morning Start Times

• Registration- 6:30am
• Shotgun start – 7:15am
• Lunch Provided – 11:30am -1:00pm

Afternoon Start Time

• Registration 11:45am-12:30pm
• Shotgun start – 12:45pm

Entry Fee: $250 per player, $1,000 per team

The outing includes: Dinner ticket for registered golfers for September 11th at Mirro Center ($50 ticket required for spouse or guest of registered golfer), Snacks and Drinks on the course, Special Gifts, Door Prizes, Low Team awards in 2 flights, Proximity awards, Hole-in-one Prizes.

Golf Sponsorship Opportunities:

• Hole Sponsors (18 total) $250 each
• Table Sponsorship for September 11th dinner $400 per table of 8
