The HOME Choice Consumer Advisory Council (HCCAC) is proud to present its fourth annual HOME Choice Conference: Breaking Down Barriers to Community Living . This year’s conference will focus on how to break down barriers consumers face when he or she transitions from a long-term care facility and into the community.

Shari Lynn Cooper
Goodwill Easter Seals of the Miami Valley
Public Relations Assistant/Motivational Speaker
Shari Cooper is a 43-year old woman with Cerebral Palsy and knows firsthand about the issues important to people with disabilities. She is a veteran advocate and a motivational speaker for people with disabilities. Cooper is described as an upbeat person who deals well with her disability, and has a natural ability to uplift anyone she comes in contact with. At a young age she became an advocate for a good quality of life for herself and her fellow peers.
While still a teenager, Shari volunteered at United Cerebral Palsy and joined the Team Program. There she was credited with changing the focus of that program from support to one of community service. She initiated a recycling drive and a nursing home visitation program. She often served as a conflict mediator for her fellow peers, and established a reputation as a dedicated advocate. Cooper always assured the best service and resources possible for people with disabilities.
In 1995, Shari graduated from Sinclair Community College with an Associate Degree in Disability Intervention Services. She was later honored by Sinclair and inducted into the Regional Developmental Disabilities Hall of Fame. Cooper was selected because of her numerous achievements in Community Service.
Shari is currently employed at Goodwill Easter Seals of the Miami Valley as a Public Relations Assistant. Her job in detail includes: Speaking to Public about the many services that are available at Goodwill Easter Seals, and giving facilities tours to the public and new employees. In 2005, Governor Bob Taft appointed Shari to the Ohio Developmental Disability Council. During that appointment she was selected as executive secretary. Shari was also nominated to Board of Directors of the National Association Councils of Developmental Disabilities in Washington, DC. She represented the great State of Ohio and addressed the concerns of all Americans with disabilities, and became Vice President of that council.
Cooper has been a member of numerous advocacy groups: Advocacy United, Advocacy Protective Services Inc., The Advocates of Montgomery County Board, Governor Council on People with Disabilities, and the State Consumer Advisory Council. Shari has an awesome family support system which includes her Wonderful Advocating Mother “Right from the Start”, four Great Brothers and her Favorite Sister which has been the backbone of her many successes including being crowned Ms. Wheelchair Ohio 2008. Shari competed for Ms. Wheelchair America and was not crowned, but instead she won a prestigious award “The Nicki Ard Achievement Award”.
Shari was also a speaker for TedxDayton 2014 where she gave the speech of her life in front of sold out audience at the Victoria Theater on, “Are You OK With Yourself”.
Shari will continue to lean on her faith while fighting for the rights of people with disabilities and advocating for a better life for all.

Due to high demand, we ask that Agencies and other organizations limit the number of people attending from their respective place to 5.
A limited number of hotel rooms are available free of charge for individuals with disabilities who either (1) Live more than 100 miles from the Conference location OR (2) Have accessibility needs that require overnight accommodations the night before the conference.
If you need a HOTEL ROOM, you must register by September 28th! If you are a professional or provider needing hotel stay, please email firstname.lastname@example.org the names of those needing rooms, and we will put you on a list to get the discounted rate. *There are a very limited amount of rooms available at the discounted rate, and priority will be given to individuals with disabilities. Rooms will be available at a first come-first-serve basis. PLEASE DO NOT CONTACT THE HOTEL FOR A ROOM! YOU MUST RESERVE THROUGH US!
Both attendant and mileage reimbursement will be available for individuals with disabilities (forms will be available the day of the conference). All requests for Reimbursements MUST be received by mail or electronically by December 12th, 2016.

What is the parking situation like?
There is valet and self-parking in the hotel's parking garage available. Valet vouchers will be given out at the conference.
