***Participation in the Illinois State Championship is contingent on the parent or guardian of each sailor agreeing to the terms of the waiver of liability below. Completion of online registration will serve as an agreement to the terms of the waiver of liability.

2015 Illinois State Sailing Championship

Hosted by Sheridan Shore Sailing School

Date: Saturday March 28, 2015 *(Saturday only)

Location: Sheridan Shore Yacht Club, Wilmette, IL

Event Chairman: Zachary Hernandez

Class: 420 (SSSS has limited boats for use), close teams will have to bring boats

**All sailors must wear a Dry Suit at all times while on the water, throughtout the event**

Registration and Entry: Competing schools must be current members of MISSA. Entries must be received by Monday March 23 at 4:30pm. Contact Zac Hernandez at (847) 251-1110 or email at sheridanshoresailingzac@yahoo.com on your intention of this regatta. Email should include school, name(s) of sailor(s), skipper and crew for A and B division. All teams must pay online.

Entry fee is $10 per competitor; lunch is not included. Out of state schools may compete in the regatta, but will not be able to compete for the IL State Title. School’s may bring more than one team, but only one of the schools may compete for the IL State Title.

9:00 a.m. - Skippers meeting

10:00 a.m. - 1st Start

Waiver of Liability

For and in consideration of Participant’s registration with the Sheridan Shore Sailing School and High School Sailing Program’s (the “Program”), and being allowed to participate in the Program, the undersigned Parent/Guardian gives permission for the above named student to participate in the Program and hereby waives, releases, and relinquishes any and all liability for and cause of action for personal injury, property damage, or wrongful death occurring to Participant arising out of, on account of, or in relation to, directly or indirectly, participation in Program events or the sport of sailing, and activities incidental thereto, wherever and however they occur, and by this Agreement any such claims, rights and causes of action that Participant may have against the Sheridan Shore Sailing School, Sheridan Shore Yacht Club, Wilmette Harbor Association, National Spiritual Assembly, Metropolitan Water Reclamation District of Greater Chicago, parent committee members, and their representatives, employees, members, volunteers and helpers for any claims of damages or injury sustained by the participant while participating in any activity related to the Program or use of the Wilmette Harbor Association or Sheridan Shore Yacht Club boats or facilities arising out of or in connection with the operation of the Program

Participant and his/her parents or guardians acknowledge, understand and assume all risks inherent in sailing and water sports activities whether on land or on water and understand that said sport and related activities involve risks to Participant’s person including bodily injury and death, and damages which may arise there from and that they have full knowledge of said risks. It is further acknowledged that there may be risks and dangers not known or reasonably foreseeable at this time. Participant and his/her parents or guardians agree to indemnify and hold harmless the Sheridan Shore Sailing School, Sheridan Shore Yacht Club, Wilmette Harbor Association, National Spiritual Assembly, Midwest Interscholastic Sailing Association, Metropolitan Water Reclamation District of Greater Chicago, parent committee members, and their representatives, employees, members, volunteers, and helpers for any claims of damages or injury sustained by the Participant while participating in any activity related to the Program or in use of the Wilmette Harbor Association or Sheridan Shore Yacht Club boats or facilities in connection with the operation of the Program.

Parent/Guardian acknowledge and agree that Coast Guard approved life jackets and shoes will be required to be worn on the docks and in the boats at all times and that the ability to swim 50 yards is a prerequisite for participation in the classes. Parent/Guardian further agrees to see to it that his or her child adheres to the Program rules. Parent/guardian further agrees to assume the obligation for the expenses of repair and/or replacement of Program equipment that is attributable to his/her child’s reckless or irresponsible behavior.

Participant and his/her parents or guardians assume full responsibility for any loss or damage, that may come to any person, boat, equipment, pier, float or other property used in conjunction with the program which loss or damage is the result of improper use, negligence, violation of the rules of any sponsoring organization, or other acts of participants, or other representatives of the sponsoring organizations, instructional program or host location in connection therewith.

IT IS THE INTENTION OF THIS AGREEMENT TO EXEMPT, WAIVE AND RELEASE THE SHERIDAN SHORE SAILING SCHOOL, SHERIDAN SHORE YACHT CLUB, WILMETTE HARBOR ASSOCIATION, NATIONAL SPIRITUAL ASSEMBLY, METROPOLITAN WATER RECLAMATION DISTRICT OF GREATER CHICAGO, PARENT COMMITTEE MEMBERS, AND THEIR REPRESENTATIVES, EMPLOYEES, MEMBERS VOLUNTEERS, AND HELPERS FOR PERSONAL INJURY, PROPERTY DAMAGE AND WRONGFUL DEATH CAUSED BY NEGLIGENCE, INCLUDING THE NEGLIGENCE OF THE FOREGOING ORGANIZATIONS AND PERSONS.

THE UNDERSIGNED ACKNOWLEDGES THAT HE/SHE HAS READ THIS AGREEMENT, UNDERSTANDS THE CONTENTS OF THIS AGREEMENT AND HAS NOT RELIED UPON ORAL OR WRITTEN REPRESENTATIONS OF THE SHERIDAN SHORE SAILING SCHOOL WHICH ARE INCONSISTENT WITH THAT CONTAINED IN THIS AGREEMENT.
