Target Audience:
Primary Care Health Home team members including the health home director, nurse care manager, behavioral health consultant, and care coordinator, clinical care team members (providers, nurses, MAs), quality improvement staff, and operations staff.
Overview:
In today's ever changing health care environment, primary care practices are being challenged to manage competing priorities, using and sorting QI and clinical data, and sustainability of PCMH along with other QI initiatives within the organization while providing high quality, patient centered care. Care teams will have the opportunity to hear from industry leaders on change management, data, QI, and sustainability. The Primary Care Development Corporation speakers will provide information regarding best practices, real world strategies for balancing and managing the triple play, and interactive hands on case examples and exercises.
The Care Team Forum is free of charge but does require registration. All travel related expenses are the responsibility of the organization.
Speakers:
Deborah Johnson Ingram is a Health Care Improvement Specialist and Program Director with Primary Care Development Corporation (PCDC) in NYC. As a subject matter expert in practice transformation and patient centered medical home, she's been able to share her expertise and consult with a variety of health care organizations to achieve NCQA PCMH Recognition across the country. In addition, with the finalization of the stage 1 CMS's EMR Meaningful Use (MU) rules, Deborah also consults on achieving both MU and PCMH 2011 simultaneously and TJC PCMH Accreditation. Deborah is a certified PCMH content expert (CCE) and she has more than fifteen years' experience in Health Care Quality Improvement and six years' experience in EMR implementation in both small and large private practice settings. Deborah was recently published in HIMSS' recent book Medical Informatics, An Executive Primer: Second Edition.
Peter Cucchiara is a senior healthcare executive with over 30 years' experience. As Managing Director, Performance Improvement at Primary Care Development Corporation he oversees all Performance Improvement services and initiatives, including care management and coordination, medical home transformation and certification, practice redesign, patient access, health information management, Meaningful Use (MU), and emergency preparedness. Under Peter's leadership the PCDC Performance improvement Practice has grown into a national operation serving a wide variety of client needs in a diverse array of primary care and community settings.
Prior to PCDC, Peter was Vice President of Information Systems for Integramed America, Assistant Director of IT as The Hospital for Special Surgery, and Quality Assurance Manager for ADP Electronic Funds Transfer Division. Peter is an active member of the College of Healthcare Information Management Executives (CHIME), Healthcare Information Management System Society (HIMSS), and the Technical Executives Networking Group (TENG). Peter received a BS in Management and Information Systems and an MBA in Information Technology from the New Jersey Institute of Technology.
